Liability Release Forms
If you have registered for a Robbie Potter clinic and have secured your spot with a deposit, please read and sign BOTH liability release forms and mail, email, or fax them to Zion Farm. If you are unable to print off documents, please contact us and we will mail them to you.
If you are registered to attend a driving clinic, you will only need to sign Zion Farm's liability release form.
*NOTE: Please mail,fax, or email a copy of the coggins for the horse(s) you plan to bring to the clinic. Your horse will not be permitted on Zion Farm without proof of a current coggins.
When dating the liability forms, please enter date for the clinic you will be participating in.
